MediPlan Medication Review · Pharmacist Iulian Nelepcu

An educational and organizational service by pharmacist Iulian Nelepcu

A licensed Romanian pharmacist reviews your parents’ full medication list.

You’re 2,000 km away. Their cardiologist prescribes, their diabetologist prescribes, their GP prescribes — and usually nobody sees the whole list. I do: I review the medicines and supplements, organise them by time of day, explain everything to your family on a 45-minute call, and give you the exact questions for their doctor. Remote, in 5 working days, €79 during the pilot.

I want the free 15-minute call No obligation. We see together whether I can help. Message me on WhatsApp I reply personally, usually the same day.

“Someone is looking at the whole list.”

The problem

Nobody looks at the whole list.

Polypharmacy — four, five, seven medicines taken every day — has become the norm, not the exception, after 65. In Romania it almost always happens in fragments: each doctor sees their own part of the picture, but nobody sees the whole picture.

On top of the medicines come the supplements from television, the teas, the painkillers taken on a hunch, and the new schedule handed over at discharge, written in a hurry on a piece of paper.

The result, in many families: the same active substance twice, wrong times of day, confusion, expired boxes in a drawer — and a son or daughter (perhaps you) carrying the worry from hundreds or thousands of kilometres away, with Google in hand, not knowing exactly what their parent takes.

Who it is for

This service is for you if:

  • you have parents aged 65+ in Romania taking four or more medicines;
  • you live abroad — or you live in Romania, caught between work, children and errands;
  • your parent was recently discharged, has a new treatment or a new doctor;
  • you have seen the medicine drawer and it worried you a little;
  • you want a professional to look, once, at the whole list — and to explain in plain words what is there.

It is not for you if you want a diagnosis, a second medical opinion or a decision about stopping a medicine. Those belong to the doctor — I prepare the conversation with them, I do not replace it.

What you get

What you receive, concretely

15 minutes to see if this makes sense

A free call, by phone or video, in which we see together whether I can help.

You send photos. That is all.

Send photos of the medicine boxes, prescriptions and discharge letters over WhatsApp. That is all. Nothing has to be perfect.

A pharmacist looks at the whole list

I go through the complete list — medicines, supplements, times of administration, overlapping active substances, what is worth clarifying.

A call your parent understands too

45 minutes with the whole family, in which I explain everything in plain language. Your parent is welcome on the call — in fact, it is best if they are there.

The report you can forward to anyone in the family

With an English summary for you: the complete medication list · what each medicine does, in simple words · a printable large-print daily schedule for the fridge · the points to clarify with the doctor · the exact questions for the next appointment · the signs that mean the doctor should be contacted.

We stay in touch — a 14-day check-in

After 14 days I come back with a message and answers to any questions that have come up about the report.

The administration schedule, printed and held on the fridge with a magnet
An example of the fridge page: black and white, large type, ticked off each day. The name and the medicines in the image are fictional.

The page that actually ends up on the fridge

The report has one page made to be printed and stuck up: no colours, large type, organised by moment of the day. Your parent ticks it off, and the family can see from far away that everything is in its place.

How it works

Five steps, with no trips to the pharmacy

  1. You book the free call (15 minutes, by phone or video).
  2. If I can help, you pay online and receive the form and the instructions for the photos.
  3. Your parent gives their consent (a simple form — I guide you through it).
  4. Within 5 working days: the family call and the PDF report.
  5. After 14 days, I come back with a follow-up.
My role: clarity, order and the right questions. Medical decisions always stay with the doctor.

What we do NOT do

The limits of the service, stated plainly

  • We do not diagnose and we do not provide medical consultation.
  • We do not recommend, stop or change treatments — only the doctor does that.
  • We do not say whether a treatment is “correct” — we flag what is worth discussing with the doctor and prepare you for that conversation.
  • This is not an emergency service. In any urgent situation, call 112 or contact the doctor immediately.
  • We do not work with oncology treatments.

We bring clarity and the right questions — decisions stay with the doctor. And you walk into the appointment knowing exactly what to ask, instead of remembering it in the car on the way back.

Who does the checking

Why I do this

I have seen the drawer. At my own parents’ home, then in dozens of families: boxes from three different doctors, a schedule scribbled at discharge, a supplement “from the TV” — and nobody looking at all of it. I am Iulian Nelepcu, a licensed pharmacist in Romania with 15 years of experience in the international pharmaceutical field and in medical communication — and I built exactly the service I needed myself.

Every case is reviewed personally by me. This is not an app and not an automated program — it is a pharmacist who takes professional responsibility for what he tells you.

Founding pilot

The Founding Programme — the first 10 families

I am launching the service with a pilot programme: €79 for the complete review, instead of €129, the standard price after the pilot. In exchange I ask for one thing: honest feedback at the end — it helps me make the service better.

You don’t fly home to check medicines. You fly home to see your parents — and only then do you open the drawer and start worrying. This can be sorted out before you get there.

€79

instead of €129 · the complete review

Paid once, not a subscription · for the whole family — for a list your parents take every single day.

  • all six components above;
  • report within 5 working days;
  • online card payment, in euros, from any country.

Only 3 places left out of 10 in the pilot programme.

What you do NOT risk:

  • the 15-minute call is free and commits you to nothing;
  • if you change your mind before the review starts, you get your money back in full;
  • medical details are collected only after you have decided, in the secure intake process — not now, not over WhatsApp, not in the contact form.

Frequently asked questions

What families ask

What is a medication review, exactly?

It is an educational and organisational service in which a licensed pharmacist reviews a person’s complete list of medicines and supplements, organises it by time of day and prepares the family with the right questions for their doctor. It is not a consultation and it does not replace the doctor.

How much does it cost?

€79 in the pilot programme, for the first 10 families; €129 is the standard price after the pilot. It is paid once, by card, in euros — it is not a subscription. The initial 15-minute call is free.

How long does a medication review take?

Five working days from receiving the complete documents. Those 5 days include the review of the list, the 45-minute family call and the delivery of the PDF report.

Does my parent have to travel anywhere?

No. Everything is done remotely: the family sends photos of the boxes and prescriptions, and the call happens by phone or video. Your parent does not leave the house and does not go to any pharmacy.

My parent is not “seriously ill”. Does this make sense?

Yes — the service is about prevention and organisation. The most useful reviews are exactly the ones done before a problem appears.

Can I not simply ask at the pharmacy?

You can, and it is a good thing to do. At the counter, though, the pharmacist has a few minutes and sees a single prescription. Here, a pharmacist goes through the whole list — medicines, supplements, habits — and you are left with a written report and a plan.

Why not ask an online assistant?

You can — and it is a start. The difference: a licensed pharmacist takes professional responsibility, knows Romanian brand names and practice, and speaks with your family. An automated program answers for nothing.

My parent is not comfortable with technology.

That is not needed. I speak with you, and the schedule arrives printed, on paper, for the fridge.

How does my parent give consent?

Through a simple consent form (signed and photographed, or given verbally and documented). Without their consent I cannot review their data — it is their right and we respect it.

What happens to the data?

We collect only what is necessary, store it securely in the EU, never pass it on, and delete it on request. Details in the privacy policy (in Romanian).

Do you also speak to my parent’s doctor?

No — I prepare you for that conversation, with the list of points and questions. The relationship with the doctor stays with the family.

What if you find something serious?

I tell you immediately and directly: “this needs to be discussed with the doctor now, do not wait for the report.”

Do you work with oncology treatments?

No, these are not part of the service.

Can I pay from abroad?

Yes — online card payment, in euros.

Someone is looking at the whole list.

The first step costs nothing and takes 15 minutes. You need no documents, no lists and no preparation — just 15 minutes in which you tell me what is going on and I tell you honestly whether I can help.
